- Create a new Approval Template
- Enable the “Declaration of Conflict of Interest” checkbox under Advanced
- Click Create
- Click “New email template field” to begin customising your declaration
- Add your field label and response options using our template below as a starting point
- Include any additional approval form questions your process requires
- Create your approval form using this enhanced template
- Add panel members as approvers to begin collecting declarations
